Drwy ddefnyddio&#8217;r Model Cartrefi Iach Pobl Iach Uwch rydym yn ceisio mynd i&#8217;r afael \u00e2 thlodi tanwydd, lleihau anghydraddoldeb iechyd y gellir ei osgoi, a gwella iechyd a lles<\/strong><\/p>\n\n\n\n<hr class=\"wp-block-separator has-text-color has-primary-color has-css-opacity has-primary-background-color has-background\"\/>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<p style=\"font-size:1.1rem\">Mae Cymru Gynnes yn gweithio mewn Partneriaeth \u00e2 <a href=\"https:\/\/www.clwydalyn.co.uk\/\" target=\"_blank\" rel=\"noreferrer noopener\">Clwyd Alyn<\/a> a <a href=\"https:\/\/www.tgpcymru.org.uk\/what-we-do\/team-around-the-tenancy\/\" target=\"_blank\" rel=\"noreferrer noopener\">Th\u00eem TGP Cymru<\/a> i ddarparu HHPLC i gefnogi o leiaf 3000 o aelwydydd sy&#8217;n agored i niwed a 5000 o drigolion ledled Gogledd Cymru i fod yn gynhesach, yn fwy diogel ac yn iachach.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"project-aim\">Nod y Prosiect<\/h3>\n\n\n\n<p class=\"has-large-font-size\">Cefnogi defnyddwyr ynni sy&#8217;n agored i niwed ledled Gogledd Cymru i fod yn gynhesach, yn fwy diogel ac yn iachach drwy ddarparu dull cyfannol sy&#8217;n cydnabod y cysylltiadau rhwng tlodi tanwydd, anghydraddoldeb iechyd y gellir ei osgoi a lles.
