Warm Wales digitally integrating Health and Housing Support -Warm Wales integrates health, housing, and social prescribing to combat fuel poverty. Starting in North Wales, they are expanding across Wales using Access Elemental Social Prescribing to enhance collaboration and data, improving public health understanding and addressing health inequalities to provide better support cohorts through effective population health management.

Many congratulations – The judges thought the winning entry was a fantastic example of a digital approach that has fostered collaboration with Health Boards, Housing Associations and many more key stakeholders.
Through enhanced data collection their approach has significantly improved the lives of many communities. Our project topped the leaderboard in this category for all 5 judges, scoring particularly well in the criteria – Demonstrates a well planned and executed initiative that harnesses digital tools effectively and Evidence of impact of the digital initiative improving the public’s health and wellbeing and/or reducing inequalities.
